Notes on the Heart-Met Miscellaneous Syndromes by 汪文綺 (Wāng Wénqǐ, Yùngǔ 蘊谷, fl. mid-18th c.)

About the work

A two-juǎn internal-medicine handbook by the Wùyuán 婺源 (Huīzhōu 徽州) physician Wāng Wénqǐ, completed c. 1754. The text comprises three zǒnglùn 總論 essays of general medical theory followed by some fifty-plus disease-specific essays covering internal-medicine, gynaecology, and miscellaneous zhèng — each accompanied by selected prescriptions and brief case-records. Doctrinally aligned with 張介賓 Zhāng Jièbīn’s (景岳) Jǐngyuè quánshū tradition, with strong emphasis on fúyáng yǎngxīn 扶陽養心 (supporting yáng, nourishing the heart) — characteristic of the Xīn’ān 新安 medical school in this period. The title is Wāng’s reflective figure: the cases and discussions represent diseases he has, over a long clinical practice, “met with the heart” — i.e. truly understood — rather than merely read about.

Abstract

Composed at the height of Wāng Wénqǐ’s clinical practice in Wùyuán and printed shortly after, the Zázhèng huìxīn lù is an exemplar of the Xīn’ān-school 新安 mid-Qiánlóng zábìng genre — Huīzhōu being one of the great regional centres of Qing literate medicine, with hereditary medical families closely intertwined with the merchant-literati elite. Two prefaces fix the date of completion: 程世法 Chéng Shìfǎ’s preface dated Qiánlóng jiǎxū = 1754 and Wāng’s nephew 吳以鎮 Wú Yǐzhèn’s preface dated Qiánlóng yǐhài = 1755. The composition is the work of Wāng’s later years, integrating decades of clinical experience inherited from his father 汪十洲 Shízhōu and elder brother 汪廣期 Guǎngqī.

The most-cited section of the work is the Fèiyōng lùn 肺癰論 (lung-abscess essay), which sets out a three-stage diagnostic and therapeutic protocol — opening stage gānjié tāng 甘桔湯 + hēidòu tāng 黑豆湯 (clearing the latent toxin and inducing eruption); suppurative stage bǎihé gùjīn tāng 百合固金湯 加減 (nourishing yīn and clearing metal/lung); post-rupture liùwèi tāng 六味湯 加減 (water-supplementing and lung-protecting). The framing of the diagnostic challenge — that lung-abscess “始萌之時最易惑人,極難識認” — has been widely cited in subsequent Lǐngnán and Jiāngnán fèibìng literature.

The work was reprinted in the Republican-period Zhēnběn yīshū jíchéng 珍本醫書集成 (Shanghai: Shìjiè shūjú, 1936), the principal modern source.
